ACTH (adrenocorticotropic hormone) is produced by the pituitary gland and stimulates the adrenal glands to produce cortisol. Normal levels effectively rule out pituitary pars intermedia dysfunction (PPID, Equine Cushing's Disease). Note: ACTH levels are normally higher in the fall (August-October) due to seasonal variation.
Low ACTH is rarely a concern in horses. It may simply reflect the individual's baseline or seasonal variation (lowest in spring).
This is not typically associated with any disease process in horses.

Equine ACTH: 32 pg/mLElevated ACTH is the primary diagnostic marker for pituitary pars intermedia dysfunction (PPID, also known as Equine Cushing's Disease). PPID is common in older horses (15+ years). Clinical signs include delayed or incomplete shedding (extra body hair), excessive sweating, weight loss, muscle wasting, increased thirst and urination (polydipsia/polyuria), and recurrent infections (especially hoof abscesses). Horses with PPID are also at increased risk for laminitis. Always interpret ACTH with seasonal reference ranges — normal fall levels are higher.
Your veterinarian will correlate this with clinical signs. The TRH stimulation test may be used for confirmation. Treatment with pergolide (Prascend) is the standard therapy and is very effective.
